WELCOME TO STUART ORAL SURGERY
Your initial appointment is a crucial step in your healthcare journey. It will involve a comprehensive consultation, during which we’ll discuss your medical history, diagnosis and treatment options. Very rarely surgery can be performed the same day as the consultation. Typically, we schedule a follow-up appointment for treatment on another day.
To streamline your consultation process, please bring the following:
- Your surgical referral slip and any relevant x-rays
- A list of medications you’re currently taking
- If applicable, complete medical or dental insurance forms to expedite claim processing
- Fill out and bring with you paper versions or submit our Patient Information form, HIPAA Consent, HIPAA Information and Medical History Form online
Note: Due to recent increases in ransomware cyberattacks we kindly ask that you do not submit new patient information to our front desk in the form of email attachments. Thank you.
For the safety and well-being of patients under 18, it’s important to note that a parent or guardian must accompany them during the consultation.
Rest assured, your safety is our top priority. A preoperative consultation and physical examination are mandatory for patients undergoing IV anesthesia. Before surgery, it’s important to refrain from eating or drinking anything for eight hours, and ensure you have arranged for an adult to drive you home afterward.
Please inform us of any medical conditions or medications you’re currently taking, such as diabetes, high blood pressure, or heart medications. This information is vital for your safety during surgery.
If your dentist or physician has taken x-rays, you can request that they forward them to our office. Alternatively, if time permits, you may collect them and bring them along. If necessary, additional X-rays or 3D scans may be taken at our facility depending on ability to accurately diagnose from previous X-rays.
